Does anyone have a suicide risk assessment form that is used at your facility, whether your facility is a correctional center, shelter, medical clinic etc? We need to implement something like this for our 3 homeless shelters.
I am in a correctional setting and we have 2 types of forms. One is simple that the officers do and the other one is more detailed that is done by the medical staff. I would be willing to fax them to you if you can give me your email address if you are still looking for something.

Actually it is must, if any one is there who can explain about the psychological state in the correctional center, then it will be nice to me. Because the suicide cases are directly dependent on the mental state of the person.
